crypto.ml_kem: avoid redundant assignment & fix K-PKE.KeyGen (#26031)

FIPS-203 algorithm 13 for deterministic key generation uses
(𝜌, 𝜎) ← G(𝑑 ‖ 𝑘) , not (𝜌, 𝜎) ← G(k ‖ d)
